Voices From Care Cymru represents the views, rights and experiences of children and young people who are or have been looked after in Wales. Our aim is to ensure that young people’s voices are heard by decision and policy makers in Wales.

Pwrpas y swydd

Bydd y swyddi yn darparu cymorth i bobl ifanc i bontio’n ddiogel o ofal.

Caiff prosiect Paratoi ei roi ar waith yn ardaloedd 3 awdurdod lleol yng Nghymru – Ynys Môn, Sir Gaerfyrddin a RhCT – i helpu pobl ifanc sy’n gadael gofal.

Bydd yn darparu model cymorth tair haen i’r bobl ifanc hynny sy’n pontio o ofal; cymorth unigol, cymorth gan gymheiriaid, cymorth yn y gymuned. Bydd hyn yn cynnwys

  • Hwyluso cymorth cofleidiol yn y gymuned
  • Cynyddu gwybodaeth pobl ifanc am eu hawliau wrth gynllunio ar gyfer gadael gofal, yn cynnwys cynyddu’u sgiliau ariannol
  • Darparu cymorth a hyfforddiant i’w galluogi i fyw yn annibynnol
  • Sefydlu amgylchiadau cyflogaeth ac addysg, a chydweithio i ddatblygu cynllun i sicrhau nad yw pontio o ofal yn tarfu ar waith, hyfforddiant a/neu addysg

Getting Ready Officer x 2 (Anglesey and Carmarthenshire)

Hours: Full time 35 hours 

Salary: £19,285 per annum

Length: Fixed Term until March 2020 (with the possibility of extension)

Office based in Cardiff & Bangor.

Purpose of post

The posts will provide support for young people to safely transition from care.

The Getting Ready project will be delivered in 3 local authority areas in Wales – Anglesey, Carmarthenshire and RCT to young people leaving care.

It will provide a three-tiered support model for those young people transitioning from care; individual support, peer support, community support. This will include

  • Facilitating wrap-around community support
  • Enhancing young people’s knowledge on their rights and entitlements when planning to leave care, including enhancing their financial capabilities.
  • Providing support and training to enable them to live independently
  • Establish employment and education circumstances and collaboratively develop a plan to ensure work, training and/or education is not disrupted by their transition from care.
